Based on MacOS X 10.11 (El Capitan), Mail 9.3 (3124)
1. Open menu Mail -> Add Account...
2. Click "Exchange" in the list of account types on the right.
3. Enter full name, email address, password and click Sign In
4. Request for further information
5. Repeat full email address for user name, and enter Internal and External URL https://webmail.prxy.com/EWS/Exchange.asmx and click Sign In
6. Select the applications to use (typically at least Mail and Contacts and Calendars, can also use Reminders and Notes) and click Done
7. In account list, you may select the account to customize it
8. Click Details... to get the account information
9. Update the account name so you can identify it easily (prxy or your company name, or anything the user wishes)
